Unable to get SlimStat working

What steps will reproduce the problem?
1. installed the plugin in wordpress
2. activated plugin
3.

What is the expected output? What do you see instead?

I don't see anything -- completely blank screen

What version of SlimStat are you using? On what operating system? Which web 
browser are you using?

SlimStat 2.8,  Wordpress 3.3.1  Firefox browser

Please provide any additional information below.

I read somewhere that I should add this bit of code somewhere:

<script type="text/javascript" src="/slimstat/?js"></script>

I'm not sure where (sorry, don't know much about code). Header? Footer? I tried 
both--still does not work

I would love to get this up and running!!
