Add module to train US-guided lumbar insertions.

Module requirements:

[x] Load anatomy models corresponding to the lumbar spine phantom
[x] Load 3D models of ultrasound probe and needle
[x] Load ultrasound probe calibration (ProbeModelToProbe and ImageToProbe), needle calibration (NeedleTipToNeedle) and phantom calibration (PatientToRas).
[x] Get tool transforms being received through OpenIGTLink (ProbeToTrracker, NeedleToTracker, TrackerToPatient)
[x] Get ultrasound image being received through OpenIGTLink (ImageReference)
[x] Create transform tree
[x] Define layouts and visualization options according to the exercise difficulty level
[x] Check US probe position during spinous process identification with transverse planes (axial)
[x] Check US probe position during interspinous space identification with the longitudinal plane (midsagittal)
[x] Check needle insertion site before insertion
[x] Check needle trajectory before insertion
[ ] Evaluate trajectory during needle insertion
[ ] Evaluate outcome. Target reached successfully?

Additional requirements:

[ ] Evaluate needle selection measuring the distance from the skin to ligamentum flavum
[ ] Check if the widest interspinous space has been selected. Widest interspinous space presents the highest likelihood for a successful procedure.

Reopening as without metrics the module cannot be considered ready.
