reported by @cstoeckert to not confuse consent codes with other type of codes, such as ICD codes

I'm not sure it's all that confusing; I like Consent Code. But if it's changed simply Consent would probably be best.

this has been updated into 'data use limitation' after discussion and approval from the DUO group
