Manual curation fixes from October

Code updates:

• Replacement terms
  • Check that replacement terms are not themselves obsolete
  • Normalise format to be URI rather than CURIE or another ID
• Exact matches - Check why exact matches in MONDO are not being always being reported
• Think about how to handle terms we know we can't map but are still meaningful terms, e.g. caused by mutation in the tbc1 domain family, member 24

Documentation updates for step 2:

• Mention possible values of ZOOMA_SOURCE (previously-used, replacement, etc.) and what they mean
• Mention to ignore the Add EFO disease tab as it is addressed in Step 4
• Clarify how to propagate Notes to the next round of curation by copying to Comments where necessary
• Remove Orphanet from importable source ontologies

Exact matches - Check why exact matches in MONDO are not being always being reported

I don't actually see an example of this as an IMPORT in the October spreadsheet, double check with the team.

This probably needs its own ticket and discussion, not addressing for Jan - #413 .
