Add mapping for baseline variable Ejection Fraction #325

The baseline field 22420 - LV ejection fraction has no concept mapping. This variable (derived from MRI) might be interesting for BigData@Heart.

We do get occurrence of Left ventricular ejection fraction | 4108582 from the GP or HES data. From baseline data we expect this measurement from around 40k persons. image
