Include unapproved in STCM export #327

There is a mismatch between actual mappings made (from resources/mapping_tables) and the ones loaded in the STCM table.

e.g.: ~4000 codes in https://github.com/EHDEN/ETL-UK-Biobank/blob/master/resources/mapping_tables/ctv3.csv and 151 'approved' codes in https://github.com/EHDEN/ETL-UK-Biobank/blob/master/resources/vocabularies/source_to_concept_map/ctv3_stcm.csv

While also unapproved codes are used.

CTV3 mapping has been updated and the two files aligned (both now contain 181512 records)
