Improve anchor link behavior and TOC

This pull request will:

Let the TOC links scroll to the exact heading position, and not somewhere above
Make sure URLs with an anchor/hash, will scroll to that anchor, instead of being blocked by the TOC. (will close #274)
Update the url with the correct anchor/hash when a link in the TOC is clicked
