Separate out metadata for physical sources and online collections

Maybe something like this? We can talk more tomorrow:

If you transcribed this from a physical copy:

Physical object (Standalone, e.g. a score of Beethoven Symphony no. 3)

Physical object (Collection, e.g. Beethoven: The Complete Symphony Collection)

If you are uploading files from another online collection or data set:

Ian's feedback also included a request for a freeform "additional information" field that could go along with this revised form
