Rewrite DissonanceIndexer for Fourths #279

Fourths are only "dissonant" in certain conditions, and the current DissonanceIndexer can't determine whether those conditions hold. Once the IntervalIndexer produces a DataFrame, the DissonanceIndexer will be able to use it to determine whether a fourth has a third "under" it.
