Institution select broken on Firefox & Safari #97

Really urgent problem:

The institution search and browse (from within the Report Form) isn't working on Firefox & Safari (but okay on Chrome). It appears to be the same or similar issue we had before. I see that the request gets a redirect back, and stops there instead of following:
