Migrate all Questionnaire instances to CIM2.

Until this is completed, 32b60ac745bca92df3d9d4036a921505d73c7edf goes some way towards linking the 2 codebases.

Actually, @murphysj is not sure if displaying "legacy" projects on the cim2 landing page is a good idea. Needs more thought.
