add support for cmeps threading #57

Add support for threading in cmeps driver.

@jedwards4b @Katetc - I don't think CISM has any OpenMP support currently. I assume these changes won't hurt, and if they enable ESMF-Aware threading, they should help CISM performance considerably in cases where other components are running threaded.
