Simply remove the two {/if} from the ending of the index.tpl.
And it works fine

I saw this error before but it went away. You've tracked down the location but 
are
slightly wrong on the fix.

if($banner_x && $banner_y}width = "{$banner_x}" height="{$banner_y}"{/if}

The problem is that the first part of the if statement is half C, half Smarty.
