Provide Mailchimp form URL #15

@nilesbarnes we need some info from you to wire up the email form.

Looks like we can use the info technical here: http://kb.mailchimp.com/lists/signup-forms/host-your-own-signup-forms to make our custom form.

But we'll need you to create a signup form within your MailChimp account. Doesn't matter much what it looks like, and should ask only for email address, but be tied to proper list in your system.

Give us the URL for the form, and we should be able to take it from there
