In #24 it's mentioned to add

What debug info a user should submit?

But I think it's best if we add it in GitHub issue template

@rahul286 I think we need a better title for this doc or I need to change its content as the content and title don't match much. If I only added about support there would be hardly few lines to add as community support would be the only option.
