prepare to send for first round of coauthor review

@ValentineHerr , I'd like to send this around to coauthors for a first pass this week in order to get their feedback on whether everything looks good for their sites. Here's what I'd like to accomplish before that:

for Valentine:

• [x] could you please read the manuscript (methods in particular) to make sure I've got everything right. There are also a few places where the methods need better descriptions.
• [x] integrate latest back-transformation
• [x] confirm that you're comfortable with the current round (recognizing that we're likely to make some tweaks, but ready to get coauthor feedback)

for Krista:

• [x] integrate comments received
• [x] add Table S2
• [x] add Table s4
• [ ] finish list of questions for coauthors (#68 )

I am re-running everything with the latest back transformation, but it won't change much and I am not too concerned about it as it is just for visualization purpose, it is fine to use the current plots for now. I have not had time to thoroughly look at all the sections of the manuscript but I am comfortable with the current round.
