Monsters failing to appear when relogging

Details

While camping Sea Hog after having killed the placeholders I logged out. I logged back in after the placeholders had respawned, but neither were visible. Then died after ghost pugils chomped me to death. No pugils on screen, no combat data in log.

Observed same issue with Yagudo when logging into the secret room on Castle Oztroja map 3. Moving well past 50 yalms away for a short time and coming back will generally cause them to appear (though sometimes without names still). Relogging while invisible sometimes corrects. Same problem if they're aggroed: they'll attack, link, and cast on you until KO with nothing showing in log or on screen.
