Add a toolbar to the UI

Add a toolbar in which basic operations are accessible

Some operations that appear to be useful

• Start Server
• Stop Server
• Switch On Fullscreen Mode
• Switch Off Fullscreen Mode
• Quit

I suggest to use the Tango Library, see http://tango.freedesktop.org/Tango_Icon_Library.

I think we have to move some things from the prefs panel to the toolbar

• enable/disable additive mode ;
• subnet and universe selection ;

I don't think that having Start/Stop server button in toolbar is a good idea : see Issue #37

maybe we should include a store cue button, and I'd like to have a more significant button for the additive mode...

Icon for additive mode changed in commit f84ccea385c990344e2c75581f194be8487cb037 I hope this will be more significant...

Before release 0.2, we have to finish the following things for this issue :
