what is considered unacceptable behavior?
how is unacceptable behavior handled?
how can the code of conduct be changed in the future?

Reframing the deliverable for this task to omit the following questions (to be answered by a deliverable for a Contribution Guide):

who may merge pull requests?
who must review a pull request before it's merged?
who may create issues?
who may close issues?
