[Instance] Utgarde Keep bugs #52

  1. No reward for finishing random dungeon.
  2. Ingvar 2nd phase in heroic is spawned as a lv 72 boss.
  3. Ingvar is casting Smash and Roar abit too often, watched some videos on youtube he'll allow some time for dps before another Smash and Roar is casted. Maybe randomize the "free time" abit?
