One of the chain quests to get to Sons of Hodir rep, The Last of Her Kind, requires you to rescue the Icemaw Matriarch from the Hibernal Cavern in Storm Peaks. You get the harness ok but the Matriarch isn't there! In fact, apart from a couple of mobs in the entrance to the cavern it's completely empty!
