Remove from GGE and focal forms

Sorry Roland I don't think I was clear - I just meant to remove the comments box but leave the 'yes' 'no' options for intellectual disability etc....

All forms now display four comorbidities without comment fields. I'll try this "reading" some time once the grant is through.
