Focal form: seizure type "Other seizure types, please specify" #37

Can you please add an additional seizure type to the focal form - "Other seizure types, please specify" with the field note "Seizures of unknown type or rare cases of generalised seizures"

And a comments box called "Other seizure types comments" please thank you!

The field is available on selection of “yes” only. Do you want me to remove the branching logic?

How it is now is great - please leave the branch logic so that people will be prompted to comment if answered yes to 'Other seizure types, please specify'.
