EE and GGE forms - Age at last occurrence #73

Based on Tony and Sam's comments, can you please change the "Age at last occurrence" field to "Date of last occurrence" for each seizure type in both EE and GGE forms. 01 for day and month can be used if the information is unknown. I hope that this isn't too much work...

It’s not a lot of work but it challenges my view how we should collect data - precise dates are HIIPA identifiers.
