New parameter to limit index patients by minimum cumulative degree #50

parameter name: part.index.degree default: 1 function: this would limit index patients in the cumulative partner list to those with a degree of part.index.degree or greater. For example, part.index.degree = 3, would remove from the list any index patients who show up in the list only 1 or 2 times.
