add current user to epidural request, record, and follow up #4

Role based access: display different front end templates for midwife vs anaesthetist. e.g. midwife user needs to see epidural request. Anaesthetist needs record epidural not request etc.

Also need to be select from a list of users if current user NOT equal to operator

performed by now auto-populates. For the MVP we will park the issue of selecting supervised by from a list of users
