Problem with two TODO.md files #61

It looks like in the last change to TODO.md it got added to the repo as todo.md so that now two copies of this file exist with the last change only being in the latter. For Linux this is not an issue but for MacOSX it is disastrous because the default is for the file system to be case insensitive. It really wrecks havoc on the local repo and I can't do anything because it thinks I changed a file that doesn't exist (because the local file system does not let me have TODO.md and todo.md in the same directory) and won't let me do anything. Help please. :)
