e-WLD: Add notes to dialect word

If an Entry has a non-empty value for 'toelichting', then add this to the dialect word in the display. That goes for the following views:

• LemmaListView: lemma_list.html
• LocationListView: location_list.html
• TrefwoordListView: trefwoord_list.html

It is not applicable to these views:

• DialectListView: dialect_list.html
• mijn_list.html
• aflevering_list.html
• admin_entry_list.html

Okay, I have added the 'dialectopgave.toelichting' to the three lists above
