Survey123 opens http://www.esri.com/legal/software-license on clicking the license agreement hyperlink in About page

@zhitaop - in both the extensions title and subheading please change it to AppStudio for ArcGIS

tested with version 0.0.7 on ubuntu 18, windows 10 and mac 10.13 and see expected logo and text
note that with the new name, it will appear as a new extension.
you need to uninstall the old one (click settings > uninstall. then click the reload button that will appear)
Note the repository and licence links at the top only appear to available after the extension is published. But we've added the standard readme content in preparation for moving to github.com/esri and this contains links to these.
