Fix an issue where Bootstrap’s validation state border styling is not visible on Calcite’s select menu controls.

As reported in #201.

On a related note, it might be good to add a control to the forms examples. Also consider adding a section showcasing validation states (similar to Bootstrap’s doc.

If the select menu’s validation state styling appears to be too light, consider changing its outline-style from inset to solid.
