Update Install Guide for 10.4

• [x] Updated software references from 10.3.1 to 10.4
• [x] Add new window service user/password variables to table in "Install" and configure the Ops Server installation scripts section.
• [x] Remove ops_install_geoevent_patches variable in table in "Install" and configure the Ops Server installation scripts section.
• [x] Remove 'Install "Portal for ArcGIS 10.3.1 Hot Fix" section.
• [x] Update footer. Change version to 10.4.0.1. Remove month and year since there isn't a release yet.
• [x] Update cover page. Change year in title to 2016. Remove "(2015 4th Quarter)" since there isn't a release yet. Update date of guilde.
