Summary: When you take enough damage to die (and have nothing else that prevents your death, modded or otherwise) you will become incapacitated and be stuck in the crawling state. Configurations can change various aspects, such as how long it takes friends to revive you, how many times you go down, and how long you have until you bleed out.
