As far as Policy-related governance sub-structures are concerned, you could probably add a Policy/RoP Management Model including a Policy/RoP Oversight Committee in the Stakeholder Forum and a Policy
Steering Committee in the Exec, or something. There are actually two functions probably: compliance with existing policy, and policy development. Compliance probably lives in the Exec but the Policy Development would perhaps have a place in both the S/H Forum and the Exec. It would be useful to know your thoughts around that, and what considerations and issues you identify in elaborating that further within the governance proposals.
