Error : Mandatory Key is missing #2

Hello,

The only two required fields listed in the info are userid and mmsid for the Excel file however I continue to get the message that a Mandatory Key is missing. I've tried adding in additional fields (item_id, request_type, request_date, title) and the file continues to fail.

I've attached the latest file and results that has all the additional fields along with the first attempt and result. Class Visit Template.xlsx results (12).xlsx

Adding pickup location type and pickup location library seems to allow the request to be process but it says no items can fulfill the request. However performing the request in Alma goes through without a problem. Class Visit Template (4).xlsx
