Add a template for PRs #1016

This is a combo of AMReX's and https://github.com/dec0dOS/amazing-github-template.

I always make a checklist for myself so I thought we might as well just make it part of all PRs. Happy to take any suggestions.

I like this template a lot! I have an ad-hoc list on my PRs too and will be good to have this as a standardized template.
