As a patient, I wish to provide my feedback after using the hospital services so that i can help the hospital to better improve their services.

Acceptance Criteria

1) Attributes : visitorName, email, phoneNumber, subject 2) Visitor Name, email address, phone number and subject must be unique 3) Visitor enters his name, email address, phone number and subject. 4) Visitor then enters his message. 5) Visit selects on "Send Us Message" to submit his feedback.
