Assessment visual updates #707

This PR:

changes AssessmentItem delete icon from mdi-close to mdi-delete
removes "double bottom" on QuestionContainer card
adds delete button next to edit button

Overall this is not something we should put a priority on 'fixing' since this is going forward a plugin 😉 That's why I haven't yet put the issue within the v5 milestone.
