Prompt: Add Health Facility Name and Module to the Edit/Mark Complete/Delete Prompt #55

Since supervisors will sometimes click on the wrong assessment when they go to mark it as completed - it would be helpful if the name of the health facility and the module would show up at the top of the prompt - like this:

Busia Health Center - 1. General OTSS Edit Mark as Complete Delete

@gtrsmith -I have moved this to E3. It is part of HNQIS, so if we really merge the apps, this feature will be there.
