I found a bug that our program may hide the extra spouse and only show the first spouse. It will be fixed.

Yeah, so blanks in spouse should be "NA" or a { }

Is there any case where one single family has more than one spouse?

The way GEDCOM organizes files is that each "couple" is organized into a separate family. For instance, re-married people will be in two spousal families.
