update pheno names #7

Updated phenotype names avoiding duplicate names. Added author and year to phenotypes that appear several times

I used gs://finngen-production-library-green/prs/PRS_data.txt as basis but looks like it's not in sync with what is in this repo - can you make sure these are in sync? Possibly best to have the ground truth in the repo so that changes can be tracked

Yeah there was a, duplicate entry in r9. I can push the r9 branch
