[md2orchestra] regression: documentation purpose #51

The <documentation> element has attribute purpose, which is typically set to "synopsis" or "elaboration" but can be empty. If the text is in a markdown table, it must have a column header, and "documentation" is used as a placeholder if purpose was not populated. However, that placeholder should not be set back in XML, but rather should result in an empty purpose.

There is also a provision for using "description" as a column header for documentation.
