Radiology Viewer: Need mockup for PACS settings modal in Library #2

When you click the 'Settings...' button highlighted in the Library search mockup snippet above, a modal should open up to let you configure the PACS you're connecting to.

This modal should have settings such as:

PACS server ip
PACS server port
called AET
calling AET
