Guardian of the Monument

After completing #1304 event, an Apexis Guardian ( http://www.wowhead.com/npc=22275/apexis-guardian#comments ) should spawn, which you have to kill in order to obtain quest item ( http://www.wowhead.com/item=32697/apexis-guardians-head ) After completing the quest it has to be delivered to http://www.wowhead.com/npc=23233/chualor

The easier way of fixing this quest would be just to implement http://www.wowhead.com/npc=22275/apexis-guardian summon into Apexis crystal without doing the scripted game event.
