1) After releasing after death in an instance, the player is almost always frozen. Yes, there are fixes to this issue such as reloading.
2) Not sure if this is connected, but after releasing after death in Patchwerk room slime, the slime continues to affect the player and kill them after resurrection.

Combat log/addons broke in Gothik room before event started
Relogged
Logged back in in front of instance portal, then went into portal and was frozen in place
