Please don't merge this yet as I'm still testing it.

This allows Koa 2 to use s3o-middleware (possibly Koa 1 as well; I haven't tested it).

Closing because I don't think this should really be in the same repo — please see forthcoming PR, which separates out most of the utilities into their own repo.
