Update chunked rendering for docbook-xslTNG 2.1.x #201

Currently, the chunked rendering is still based on docbook-xlsTNG 1.5.4. It needs to be updated to work with docbook-xslTNG 2.1.x. We probably need to redo the custom CSS in src/theme/docbook5-css, and maybe some of the parameters in src/docs/xsltng need to be changed.

I have deployed updated documents. There is an issue with the persistent toc not scrolling to the right section, reported at https://github.com/docbook/xslTNG/issues/451. I have a workaround/fix for this, but I haven't deployed that yet.
