Add core extensions package to FhirPackageSource #50

Once R5 is released, we need to update the default dependencies to also allow the new core extensions packages.

When moving the package source to this project, we removed the core package sources (because the lib is verison independent). We should add a function to make this easier for the user (and also include the extensions package).
