First PR to avoid one bit PR for refactor and making demo projects easier to work with.
Moved projects, files and renamed for consistency in prep for making demo more usable for demonstration and cql development examples.
Moved CS files to subfolder CSharp in measures projects
Renamed Measures projects with dot notation
Renamed Measures project to Measures.Demo to reflect its contents
Renamed Test project to Test.Measures.Demo to reflect its relationship
Future PR will create new project specifically for measure creation so CSharp child was needed for better organization as other folders will soon exist, input/cql, elm, resources, etc
Leaving resources for "testing" projects in LibrarySets for now. Doesn't look like these need to move as I don't see a clear need for really modifying these if there is a new project sample for CQL development work.
