Details: This isn't persistant, but frequent enough to be somewhat fun spoiling. NPCs sporadically all lose their awareness of everything around them seemingly, resulting in the player being able to run right up to them before the NPC becomes aware of them -WITHOUT STEALTH-. As in, standing.
This is an additional problem for followers, because frequently, when the player enters combat, the follower, if they're some distance behind, doesn't 'see' the enemies and doesn't engage. Furthermore, if the player is downed, and the follower hasn't caught up, it can result in the enemies entering a sort of "search" mode and the game soft-locking as the player is stuck in purgatory waiting for their follower to be defeated, or win.
