Answer: Attached. I can confirm this issue is happening with 2 separate new characters.

Answer" Randomly, my screen will go blank and the game will halt. This actually locks the entire PC. I can CTRL/ALT/DEL but am unable to ALT/TAB to it to kill the process. I ultimately have to "sign off" system to resolve each time it happens. So far, it happens every few minutes. (side issue: I did have a scenario with my first character setup to choose the vampire alt start. When I attempted to put on the vampire armor, it was invisible. this only happened with that 1 type of armor)

Answer: Its occurred numerous times, all random in my playthrough.

Answer: None available as screen just goes black and Ctr/Alt/Del shows Skyim as "Not Responding", so its a hard lock. But it never returns to desktop. Again, i have to sign out to clear this.
